Case Manager II - Medicare & Waivers
- Reports To: Program Manager - Specialized Housing Stability (LICSW)
- Work Schedule & Location: This is an in-office position, with flexibility for occasional remote work or adjustments to daily schedule as operational needs allow. Core working hours fall between 8:00 a.m. and 6:00 p.m., with flexibility in start and end times.

PositionOverview
The primary objective of the Case Manager II - Medicare & Waivers is to support Minneapolis Public Housing Authority (MPHA) residents across the lifespan, including families with dependent children, older adults, and people with disabilities. This position assists residents in accessing and maintaining health coverage, long-term services and supports, waiver services, and other benefits that promote independence, support aging in place, and strengthen housing stability.
The Case Manager II - Medicare & Waivers provides resident-centered case management and benefits navigation with a focus on Medicare, Medical Assistance (Medicaid), MnCHOICES, home and community-based services, disability-related supports, and long-term services and supports. The position assists residents and families with referrals, applications, renewals, verification requirements, notices, and coordination with county, health plan, and community partners. This may include support related to CADI, CAC, DD and BI waivers; Alternative Care; Community First Services and Supports (CFSS); TEFRA Medical Assistance; and other applicable programs. Services are provided primarily at MPHA properties in collaboration with residents, families, MPHA staff, Agate staff, and external providers.
PositionResponsibilities
- Provide individualized case management and benefits navigation to MPHA residents and families across the lifespan, with a focus on Medicare, Medical Assistance (Medicaid), waiver services, disability-related supports, and long-term services and supports.
- Assist residents and families with health coverage and benefit applications, renewals, verifications, eligibility notices, required documentation, and follow-up steps, including Medical Assistance and Medicare-related benefits as applicable.
- Complete and coordinate MnCHOICES referrals; help residents and families prepare for assessment and reassessment processes; and support follow-up with county staff regarding service eligibility and next steps.
- Support residents in navigating applicable waiver and community-based service pathways, including CADI, CAC, DD and BI waivers, Alternative Care, and Community First Services and Supports (CFSS), while helping residents understand service options and provider choices.
- Support families with dependent children in identifying and navigating disability-related benefit and service pathways, including TEFRA Medical Assistance, CFSS, applicable home and community-based waiver programs, and other family support resources such as the Family Support Grant when appropriate.
- Coordinate with county staff, health plans, care coordinators, assessors, service providers, schools or family-serving partners when appropriate, and community organizations to support timely access to benefits and services.
- Support residents and families in gathering records, completing required forms, responding to verification requests, and completing follow-up steps needed to maintain coverage and avoid preventable gaps in benefits or services.
- Identify barriers affecting health, disability-related support needs, independence, caregiving, or housing stability and connect residents and families to appropriate community resources and supports.
- Provide follow-up with residents and families to monitor progress and address changes in eligibility, coverage, service needs, household circumstances, or care arrangements.
- Maintain regular onsite presence at assigned MPHA properties; build collaborative relationships with MPHA staff, resident leaders, Agate staff, families, and community partners; and provide services in locations that are accessible and convenient for residents.
- Complete accurate and timely case notes, data entry, benefit tracking, referrals, application status updates, releases of information, and other required documentation while maintaining confidentiality and professional boundaries.
- Maintain working knowledge of Medicare, Medical Assistance, MnCHOICES, Minnesota waiver and long-term services programs, CFSS, TEFRA, and related eligibility processes; participate in training as systems and policies change; and advocate with and on behalf of residents while supporting resident choice, self-determination, and independence.
- Performs all other duties, as assigned.
RequiredQualifications
- Bachelor's degree or minimum 2 years of experience in case management, benefits navigation, disability services, health and human services, or a similar role.
- Experience assisting individuals or families with public benefits, health coverage, disability services, long-term services and supports, or service eligibility processes.
- Ability to understand and explain complex health coverage, waiver, assessment, and service-system information in a clear, respectful, resident- and family-centered manner.
- Proficiency with Microsoft Office Products.
- Excellent problem solving and conflict resolution skills.
- Proficiency in multi-tasking while maintaining accuracy.
- Must have valid MN Driver's License and pass background check.
- Demonstrated ability to network via email, telephone, and in person to advocate for participants.
This position requires use of your own personal vehicle and will require insurance liability coverage of at least one hundred thousand dollars per person and three hundred thousand dollars per occurrence and must have a valid Driver's License and pass a driving record background check.
PreferredQualifications
- Experience working with older adults, people with disabilities, and/or families with dependent children in housing, health, disability, community-based, or social service settings.
- Experience with Medicare, Medical Assistance (Medicaid), MnCHOICES, Minnesota home and community-based waiver programs, CFSS, TEFRA, or other long-term services and supports.
- Experience coordinating with counties, health plans, care coordinators, MnCHOICES assessors, waiver providers, schools, or other human service systems, including support with referrals, applications, renewals, reassessments, appeals, or verification processes.
- Experience working in public housing, affordable housing, supportive housing, or resident services and building collaborative community relationships is preferred. Bilingual skills are preferred but not required; knowledge of Housing First, Harm Reduction, person-centered practice, family-centered practice, and resident choice is also preferred.
