As a Case Manager II in our Young Parent Support (YPS) Program, you’ll work directly with expectant and parenting young people ages 14–24, helping them create safe, stable, and nurturing lives for themselves and their children. You’ll build meaningful relationships with young parents in their homes and communities while helping them navigate services, reach their goals, and build the skills and confidence they need to thrive.

What You’ll Do:

  • Provide strength-based, trauma-informed case management and support to pregnant and parenting youth ages 14–24.
  • Build trusting relationships with young parents through regular home and community visits.
  • Work alongside participants to identify their goals and develop individualized service plans that reflect their priorities and strengths.
  • Help young parents access resources such as childcare, housing, education, employment, health care, WIC, Early Intervention, and behavioral health services.
  • Provide parenting education, coaching, and practical support using evidence-informed approaches and curricula.
  • Help young parents develop parenting, life, and self-sufficiency skills that support long-term stability and independence.
  • Coordinate care with DCF workers, schools, healthcare providers, childcare agencies, and other community partners.
  • Advocate for participants while supporting their voice, choice, independence, and ability to make decisions about their own lives.
  • Track goals and progress through regular visits, communication, and documentation.
  • Maintain accurate and timely case records in accordance with agency and funder requirements.
  • Participate in team meetings, professional development, program activities, and required data collection and reporting.
