Job description

As a Case Manager, you will play a vital role in supporting youth as they work toward achieving their personal, educational, and treatment goals. You will be responsible for developing, coordinating, and facilitating individualized case plans and treatment programs that meet the requirements of the referring agency while addressing the unique needs of each youth on your caseload. Working closely with a team of dedicated professionals, you will help create meaningful opportunities for growth by providing individual counseling, teaching essential life skills, and guiding youth through their treatment journey.

In this role, you will build positive relationships with youth, families, probation officers, referral agencies, and other key stakeholders, ensuring open communication and ongoing updates regarding each youth’s progress. You will foster a safe, supportive, and encouraging environment where youth feel valued and empowered to succeed. Additionally, you will monitor and document clinical and behavioral progress while actively participating in educational, social, recreational, and athletic activities that promote personal development, confidence, and long-term success.

To be considered, you should: a High School Diploma or GED is required. Bachelor's degree in human services preferred. At least one year experience working with adolscents is preferred. Ability to pass a criminal background check, drug screen, and TB test. Ability to effectively communicate, verbally and in writing. Must be able to maintain a high level of confidentiality. Ability to work in excess of 40 hours per week as needed. Must be flexible and able to handle multiple priorities.
