About the Role

As a Permanent Supportive Housing Case Manager, you'll play an important role in helping individuals and families living with HIV/AIDS find and maintain safe, stable, and affordable housing.

Through our Housing Opportunities for Persons With AIDS (HOPWA) Tenant-Based Rental Assistance (TBRA) program, you'll work directly with clients to navigate housing challenges, access rental assistance, connect with healthcare and supportive services, and build a foundation for long-term stability.

This is a relationship-driven role where compassion meets problem-solving. You'll get to know your clients, understand their unique circumstances, and work alongside them to create individualized plans that support their housing, health, and personal goals.

Your days will be a combination of client meetings, housing coordination, home visits, documentation, and collaboration with landlords, healthcare providers, and community partners. Some days you'll help someone navigate a housing challenge; other days, you'll connect a client with essential healthcare or celebrate an important milestone in their journey toward independence.

What You'll Do

  • Provide individualized case management services to clients, helping them identify barriers, set meaningful goals, and work toward long-term housing stability and self-sufficiency
  • Conduct comprehensive client assessments and develop personalized service plans based on each individual's housing, health, and supportive service needs
  • Administer Tenant-Based Rental Assistance (TBRA) in accordance with program guidelines and organizational policies
  • Help clients navigate housing challenges and coordinate rental assistance with landlords, property managers, and service providers
  • Build positive working relationships with clients, landlords, healthcare providers, and community partners to support successful housing outcomes
  • Connect clients with appropriate healthcare, community resources, supportive services, and other essential assistance
  • Coordinate referrals and follow up to help ensure clients receive the support they need
  • Conduct required home visits and client check-ins to monitor progress, identify emerging needs, and promote continued housing stability
  • Provide ongoing encouragement and practical support as clients work toward greater independence and self-sufficiency
  • Help clients navigate concerns or challenges that may threaten their housing stability
  • Use trauma-informed, culturally responsive, and nonjudgmental approaches when supporting clients through difficult situations
  • Recognize crisis situations and respond appropriately using stabilization, safety planning, and available community resources
  • Maintain accurate, thorough, and confidential client records in required program systems
  • Document case management services, client interactions, housing assistance, and program activities according to established guidelines
  • Complete required reports, forms, and program documentation accurately and within established deadlines
  • Collaborate with supervisors, internal teams, and external service providers to address complex client needs and identify creative solutions
  • Participate in required program meetings, trainings, and organizational activities
  • Represent The Just One Project at a minimum of six Pop Up & Give events each calendar year
  • Support additional program and organizational responsibilities as needed

What You Bring

  • Bachelor's degree in Social Work, Human Services, or a related field, or an equivalent combination of education and relevant work experience
  • 3+ years of experience in case management, social services, or community outreach
  • 3+ years of nonprofit experience
  • Experience working with a caseload of 20-25 people.
  • Experience working directly with diverse populations, including vulnerable or at-risk individuals
  • Knowledge of HIV/AIDS-related issues, permanent supportive housing assistance programs, and available health resources
  • Familiarity with housing services, wraparound programs, and initiatives that promote long-term self-sufficiency
  • Demonstrated experience managing a caseload and helping clients achieve measurable outcomes related to housing stability and independence
  • Strong communication and interpersonal skills, with the ability to establish trust and maintain professional relationships
  • A compassionate, client-centered approach that respects each individual's circumstances, experiences, and goals
  • Strong organizational skills and attention to detail, especially when managing multiple clients, deadlines, and documentation requirements
  • Sound judgment and creative problem-solving skills when navigating complex housing or client situations
  • The ability to maintain strict confidentiality when working with sensitive personal and health-related information
  • Knowledge of crisis stabilization, rapid response, and safety planning practices
  • Comfort using technology and client management systems to maintain accurate case records and program documentation
  • The ability to work independently while collaborating effectively with teammates and community partners
  • A valid, unrestricted driver's license, reliable transportation, and the ability to meet organizational insurability requirements
  • The ability to obtain and maintain a valid Nevada Food Handler Safety Training Card within 30 days of hire when required for food distribution activities

Preferred Qualifications

  • Relevant certifications in case management, conflict resolution, or related areas
  • Bilingual language skills

What the Work Looks Like

This is a full-time, onsite position based at The Just One Project's headquarters, with regular opportunities to work directly in the Southern Nevada community.

This isn't a position where you'll spend every day behind a desk! You'll balance administrative responsibilities and case documentation with client interactions, community-based services, and housing-related activities.

Because local travel is an important part of this role, you'll need to be comfortable:

  • Traveling throughout the community for client home visits, meetings, and other program-related activities
  • Working independently in office and community-based settings
  • Visiting community locations, partner agencies, and other organizational facilities as needed
  • Sitting, standing, and walking for extended periods
  • Occasionally bending, reaching, stooping, lifting, or carrying materials up to 50 lbs., when applicable
  • Working in varying indoor and outdoor environments
  • Responding professionally and calmly when unexpected challenges arise
