We are seeking a compassionate and skilled RN Admission Care Liaison to join our South Region team, serving the greater Miami Valley and surrounding communities.

  4. Position Details

    • Location: Field-based – South Region / Greater Miami Valley and surrounding communities
    • Schedule: 11:00 a.m. – 9:30 p.m. day/evening shift
    • Weekend Requirement: Every third weekend
    • Holidays: 1–2 holidays per year
    • Travel: Regular driving throughout the service area and to various patient care settings

    Candidates must be comfortable traveling independently and driving during daytime, nighttime, and adverse weather conditions.

    About the Role

    The RN Admission Care Liaison plays an essential role in helping patients access hospice care when they need it most. You will complete hospice admissions, assess patient needs, coordinate services, educate patients and families, and connect them with the resources and support they need throughout the end-of-life journey.

    The ideal candidate brings strong clinical judgment, excellent communication skills, compassionate patient advocacy, and a genuine passion for hospice and palliative care.

    Key Responsibilities

    • Complete hospice admissions accurately, efficiently, and compassionately.
    • Perform comprehensive assessments and determine appropriate hospice services.
    • Educate patients and families about hospice philosophy, services, and available resources.
    • Coordinate care with patients, families, referral sources, healthcare providers, and community partners.
    • Build and maintain positive professional relationships with referral sources and healthcare partners.
    • Document patient assessments, interactions, and services accurately in the electronic medical record at the point of care.
    • Meet organizational performance expectations, including maintaining a 75% admission conversion rate.
    • Follow all applicable hospice regulations, organizational policies, and standards of care.
    • Advocate for patients and families while helping ensure a smooth transition to hospice services.

    Required Qualifications

    • Graduate of an accredited nursing program.
    • Active, unrestricted Registered Nurse (RN) license in the State of Ohio.
    • Current CPR certification.
    • Strong computer skills and proficiency with electronic medical records.
    • Valid driver’s license, reliable transportation, and a safe driving record.
    • State-required minimum automobile insurance coverage.
    • Ability to travel throughout the service area and drive during daytime, nighttime, and adverse weather conditions.
    • Ability to successfully complete a background check and 10-panel drug screening.

    Preferred Qualifications

    • Associate Degree in Nursing (ASN) or Bachelor of Science in Nursing (BSN).
    • Two years of acute care nursing experience.
    • Hospice and Palliative Nursing (CHPN) certification.

    CHPN certification is preferred and must be obtained within two years of employment. Eligibility begins after completing 500 hours of hospice or palliative care experience within the previous 12 months.
