Moab Regional Hospital is a 17 bed, non-profit, Critical Access Hospital in Moab, UT seeking a RN Staff Nurse to provide services for the Moab Regional Recovery Center as well as throughout the Moab Regional Hospital Healthcare System. General Job Description:An experienced Registered Nurse (RN) with expertise in addiction nursing, psychiatric care, MOUD pharmacotherapy, and clinical pharmacy dispensing is required to operate the clinical dosing dispensary within the Opioid Treatment Program (OTP) at the Moab Regional Recovery Center (MRRC). The RN Pharmacy Dosing Nurse holds primary clinical responsibility for the daily, safe, accurate, and low-barrier dispensing, administration, and accounting of Medications for Opioid Use Disorder (MOUD)—specifically liquid Methadone and sublingual Buprenorphine formulations—pursuant to authorized practitioner medical orders in the SMART Electronic Health Record (EHR). Operating within a co-located, bidirectionally integrated behavioral and physical healthcare setting, this position combines precision medication reconciliation with compassionate pre-dosing clinical triage, observed ingestion monitoring, take-home dose preparation, and non-punitive safety holds. Under the administrative direction of the Program Director, the medical supervision of the Medical Director, and the clinical pharmacy direction of the Licensed Consulting Pharmacist, the Dosing Nurse ensures all dispensary operations maintain an airtight compliance audit trail complying strictly with Drug Enforcement Administration (DEA) regulations (21 CFR Chapter II), SAMHSA standards (42 CFR Part 8), HIPAA, 42 CFR Part 2 federal privacy covenants, Utah Division of Licensing and Background Checks rules (R501-1, R501-21, R380-600), and 2026 CARF Accreditation Standards.Essential duties, skills andresponsibilities
- Medication Administration & Observed Ingestion: Prepares, dispenses, and administers MOUD (methadone liquid and sublingual buprenorphine) strictly pursuant to individualized, signed prescriber orders (MD, APRN, PA) in the SMART Electronic Health Record (EHR). Verifies patient identity using valid government-issued photo ID prior to dispensing. Executes strict observed ingestion protocols at the dosing window, ensuring medication is completely swallowed and requiring the patient to speak and drink water before leaving to eliminate dose cheeking or window diversion.
- Physical Controlled Substance Inventory & DEA Security: Conducts mandatory daily double-signature physical sight hand-counts of active, bulk, and quarantined Schedule II and III controlled substance inventories at every shift handoff, reconciling counts against electronic MAR dosing reports. Participates in receiving incoming shipments, verifying manufacturer tamper-evident seals, and securing inventory immediately inside alarm-protected, double-locked safes/vaults. Maintains an airtight audit trail under 21 CFR § 1304.24 and manages quarantine safe compartments for expired, damaged, or recalled stock.
- Clinical Pre-Dosing Screening & Impairment Safety Holds: Evaluates patient physical demeanor, cognitive stability, pupil response, and speech prior to dose administration. Conducts pre-dosing clinical assessments to identify signs of acute central nervous system (CNS) depression, alcohol/sedative intoxication, or severe withdrawal. Immediately executes temporary dosing safety holds when impairment or medical instability is observed, escalating the case to the Medical Director or Prescribing Practitioner for clinical review before issuing any dose.
- Take-Home Medication Preparation & Safety Check-ins: Prepares authorized take-home medication doses in system-generated, child-proof containers featuring accurate, complete take-home labels meeting state and federal standards. Verifies that patients possess an approved, locked medication box prior to handing over unsupervised doses. Coordinates and executes unannounced take-home callback safety check-ins, inspecting bottle seal integrity and performing exact physical dose reconciliations.
- Dispensary Calibration, Hand-Pumps & Chemical Deactivation: Performs daily automated pump calibrations and accuracy test draws using graduated volumetric cylinders prior to opening the dosing window. In the event of power or IT system outages, seamlessly pivots to manual, emergency hand-pump dispensing in strict accordance with clinical procedures. In cases of medication spills, contamination, or patient emesis, executes immediate chemical deactivation and neutralization, logging all inventory variances in the SMART EHR within 24 hours.
- Electronic Health Record (EHR) & MAR Documentation Integrity: Maintains real-time electronic MAR entries for every administration, dose change, or hold (paper logging is strictly prohibited under the EHR-Only Logging Standard). Documents complete nursing assessments, vital signs, TB screenings, and clinical triage observations using trauma-informed, destigmatizing, and objective language (e.g., 'urine positive for substances' instead of 'dirty tox').
- Interdisciplinary Care Coordination & Team Staffing: Actively participates in weekly Treatment Team case conferences, collaborating with medical prescribers, clinical counselors, case managers, and peer support specialists to review patient stability, adjust care plans, and coordinate care. Participates in monthly clinical pharmacy site reviews with the Licensed Consulting Pharmacist and quarterly direct clinical observations with the Medical Director.
- Laboratory Collection & Communicable Disease Screening: Administers baseline health screenings, collects random toxicology specimens under non-punitive harm-reduction protocols (minimum 8 random tests per year per patient), conducts pregnancy testing for individuals of childbearing potential, and administers/reads Tuberculosis (TB) screenings utilizing the standardized OL-TB Screening Tool.
- Privacy, Regulatory Compliance & Community Relations: Strictly enforces federal 42 CFR Part 2 and HIPAA privacy covenants, protecting patient records and preventing unauthorized disclosures. Conducts daily Central Registry (Lighthouse) dual-enrollment checks and removes terminated staff credentials. Greets patients with dignity and respect, de-escalating lobby distress and maintaining a welcoming, therapeutic clinic environment.
- Departmental Cross-Training & Role Flexibility: Demonstrates total flexibility and readiness to cross-train across MRRC nursing functions—including admissions triage, outpatient clinical nursing, case management coordination, and Mobile MAT Unit support—ensuring uninterrupted clinical coverage during peak volume or staff absences.
Minimum Qualifications
- Academic Credentials: Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) from an accredited college or university.
- Professional Licensure: Active, unrestricted Registered Nurse (RN) license in the State of Utah (or an active Nurse Licensure Compact - NLC multistate license). Primary Source Verification (PSV) of licensure directly through DOPL is mandatory prior to direct service delivery.
- Clinical Experience: Minimum of one (1) year of direct clinical nursing experience required. Prior experience in addiction medicine, opioid treatment programs, psychiatric nursing, clinical pharmacy dispensing, or emergency triage is highly preferred.
- Cardiopulmonary Resuscitation: Current Basic Life Support (BLS) / CPR and First Aid certification required.
Requirements
- Background Clearance: Must pass a Utah Direct Access Clearance System (DACS) criminal background check and fingerprinted Rapback enrollment pursuant to Utah Admin. Code R501-1 and R501-14. If hired conditionally, must operate under a signed Pre-DACS Direct Supervision Plan until final state clearance is issued.
- Health Clearance: Must complete pre-service Tuberculosis (TB) screening clearance utilizing the standardized OL-TB Screening Tool and pass a pre-employment drug screening.

Requirements
Must be at least 21 years of age. Must possess a valid driver's license and acceptable Motor Vehicle Record (MVR) if designated to drive for clinic business or the Mobile MAT Unit (MMU). Note: Direct passenger transportation of clients in personal or hospital vehicles is strictly prohibited. - Pharmacotherapy & Addiction Science Knowledge: Thorough understanding of OUD pathophysiology, MOUD pharmacotherapy (methadone, buprenorphine, naltrexone), withdrawal management scales (COWS, SOWS), dosage calculation mathematics (mg/mL conversions), and aseptic dispensing techniques.
- Technical & Software Literacy: Proficient in operating computer terminals, Microsoft Office (Word, Excel, Outlook), MCN Policy Manager, Relias LMS, Meditech, and the SMART Electronic Health Record system.
- Trauma-Informed Communication: Demonstrates exceptional interpersonal, active listening, and conflict de-escalation skills. Maintains a non-judgmental, person-first attitude, respects patient autonomy, and enforces strict professional boundaries in a close-knit rural community setting.
- Standing or sitting for extended periods at the dosing window terminal and EHR computer workstation.
- Fine motor dexterity required to operate automated dispensing pumps, calibrate hand-pumps, measure liquid medication volumes, open child-proof container seals, and execute rapid keyboarding.
- Visual acuity necessary to inspect medication bottle labels, verify dose measurements, read computer monitors, and observe subtle physical/behavioral signs of patient impairment or distress.
- Ability to lift light loads (up to 25 lbs) when receiving and moving secure medication shipments or handling patient lockboxes.
- Ability to communicate clearly, interpersonally and via telephone/video technology, with patients, physicians, and multidisciplinary team members in a secure clinical dispensary setting.
