The Marion Il VMAC is seeking a?full-time?registered nurse to join the Paducah CBOC Primary Care Team

Registered Nurse Case Manager provides patient-centered care management principles to support Veterans in managing chronic, high-risk, or post-acute conditions. The RN performs comprehensive assessments, develops individualized care plans, implements clinical interventions, and evaluates patient outcomes across the continuum of care. The RN triages patient-generated data-including vital signs, symptom reports, and structured clinical questions-identifying potential exacerbations and initiating timely interventions. The RN collaborates extensively with interdisciplinary partners, including PACT, HBPC, Mental Health, Social Work, Pharmacy, and Nutrition, and facilitates communication with community providers when care is shared. The RN delivers health education, coaching, and motivational counseling to enhance Veterans' self-management skills, reduce preventable hospitalizations, and improve access to care. The RN adheres to all organizational policies, clinical guidelines, regulatory requirements, and demonstrates beginning leadership skills and sound clinical decision-making in providing safe, effective telehealth nursing care VA offers a comprehensive total rewards package: VA Nurse Total Rewards Pay: Competitive salary, regular salary increases, potential for performance awards Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Licensure: 1 full and unrestricted license from any US State or territory Work Schedule: Monday - Friday 8:00am-4:30pm Telework: not available Virtual: This is not a virtual position.
