Job Description
About the Position: This position is responsible for crisis evaluations, preauthorization requests, and coordination of admissions to Cedar Springs. The position consists of answering phone calls, conducting mental health evaluations, evaluating for involuntary commitments, making clinical recommendations for optimal level of care referrals and community adjustment, coordinating with community providers and referral sources, and facilitating internal admissions to Cedar Springs programs. Needs to perform vitals, breathalyzer tests as well as temperature on each potential patient at time of evaluation. Immediate consideration for overnight schedules.

Cedar Springs Hospital COVID-19 vaccination mandate:
In accordance with Colorado state public health order, all healthcare workers must be vaccinated, and the Company must verify vaccine status. Pursuant to this public health order, the following modes constitute as proof of vaccination:
- an electronic entry in the Colorado Immunization Information System (CIIS); OR
- an immunization record from a licensed healthcare provider who has administered a COVID-19 vaccine to an individual who provides services for the facility or agency, specifying the vaccine administered, name and title of the person who administered the vaccine, address of the location where the vaccine was administered, and the date it was administered.
A COVID-19 Vaccination Record Card (issued by the Department of Health and Human Services Centers for Disease Control & Prevention or WHO Yellow Card) which includes name of person vaccinated, type of vaccine provided, and date last dose administered would suffice.
Exemptions
- Workers may be exempt from the vaccination requirements by providing a signed exemption form indicating one of the following:
- Religious Beliefs
- Qualifying Medical Reason
- To be eligible for an exemption based on Religious Beliefs, the worker must provide a written statement describing the worker’s sincerely held religious beliefs and how those beliefs conflict with Colorado’s August 21, 2021 Public Health Order requiring certain healthcare workers to be fully vaccinated against COVID-19. The company may request supporting documentation to further evaluate the request for religious accommodation.
- To be eligible for a Qualified Medical Reason exemption, the worker must provide a written statement signed by a physician, physician assistant, advanced practice nurse, or certified nurse midwife licensed in the State of Colorado stating that the COVID-19 vaccination for the employee, direct contractor, or support staff is medically contraindicated as described in the product labeling or authorized by the FDA. This statement should not describe the underlying medical condition or disability but should indicate the probable duration of the inability to receive the vaccine.
- A record of signed exemption forms and statements submitted in support of the exemption request must be maintained in a confidential HR Medical file, separate from the Personnel File.
