Our Independent Living Group Homes are designed to prepare adolescents for self-sufficiency by teaching essential life skills to youth who are aging out of the foster care system.
Job Overview
The Behavioral Health Case Manager is a key member of the Behavioral Health Team, responsible for supporting and guiding youth as they transition into adulthood. This includes developing service plans, teaching life skills, coordinating care with service team members, and ensuring documentation and regulatory compliance. The BH Case Manager works closely with youth, Behavioral and Group Home Teams, and other support teams to promote stability, independence, and long-term success for residents.
Key Responsibilities
- Support youth through individualized case management: Conduct assessments, create and update service plans, and lead regular review meetings to keep each youth’s goals on track.
- Teach independent living skills: Provide hands-on guidance in budgeting, education support, job readiness, household management, health, and relationship-building through both one-on-one and group sessions.
- Provide direct support and supervision: Ensure youth’s daily needs are met, supervise them in activities at the home and in the community, and coordinate their participation in school, work, appointments, and other commitments.
- Maintain accurate records: Keep service plans, progress notes, and other documentation up to date, ensuring compliance with agency policies and state/federal standards.
- Collaborate with partners: Serve as an advocate by working closely with group home staff, clinical teams, schools, employers, and other service providers to support each youth’s success.

Required Skills & Qualifications
- At least 21 years of age
- High school diploma or GED
- Valid OH driver’s license, reliable transportation, and ability to meet agency driver requirements
- Must be able to pass varying background checks, included but not limited to BCI and FBI
- Minimum of 1-3 years of experience in behavioral health case management or a related field.
- Knowledge of mental health and co-occurring disorders, crisis intervention techniques, and community resources.
- Strong interpersonal and communication skills, with the ability to work effectively with clients, families, and interdisciplinary teams.
- Ability to work independently, manage a caseload, and prioritize tasks in a fast-paced environment.
- Proficiency with electronic health records (EHR)
- Experience working with youth and a basic understanding of independent living skills
- Excellent oral and written communications skills
- Must be able to be trained in and comprehend trauma informed care and how to deal with complex childhood trauma
Preferred Skills & Qualifications
- Familiarity with OAC and CARF standards related to group home care
Schedule & Location:
- 8 hour shift
- 12:00 PM – 8:00 PM
- Monday to Friday
- Hybrid work location – Some administrative tasks may be performed remotely; however, in-person work at our Mansfield or surrouding area group homes is a regular and essential part of the role.
Job Type – Full-Time, Regular, Exempt
