Presbyterian Homes of Georgia is currently seeking a caring individual to support the Nursing and Health Services areas by performing a variety of tasks as a Care Coordinator. Duties of the Care Coordinator include providing administrative support to the Nursing and Health Services areas, assisting in daily office needs, coordinating the office's general activities all while maintaining the highest levels of confidentiality.

The Director of Social Services plays a vital role by leading the day-to-day operations of the Social Services department. This position will be responsible for ensuring that the emotional and psychosocial well-being of all residents are being met, as well as ensuring that all necessary documentation is completed and maintained.
The ideal candidate should be an experienced social services professional with strong interpersonal and people skills who can effectively and professionally engage with the residents, family members, the care team and the community as a whole.
The successful candidate will be highly collaborative, and partner successfully with the Executive Director, as well as other members of the campus leadership team to ensure that excellence in all aspects of the social services area are provided and maintained.
Responsibilities (Overview):
- Ensures that the emotional and psychological well-being of all residents are met.
- Ensures that all necessary and required documentation is completed timely and maintained appropriately.
- Engages with residents, family members, members of the care team to ensure that needs are met and quality services are provided.
- Attends new admissions meetings, as well as new resident move-in activities.
- Attends Care Plan meetings/conferences.
- Works with residents, families and care team members to ensure any concerns are addressed and resolved timely and appropriately.
- Assists with communication to family members as necessary.
The ideal applicant should have the following minimum qualifications:
- A Caring Heart and a Passion for working with Senior Adults.
- Undergraduate degree in Social Work.
- Licensed Social Worker (LSW) is preferred.
- A minimum of two (2) years of experience in social services in a skilled nursing facility is required.
- Strong & solid knowledge of both Skilled Nursing and Assisted Living regulations.
- Certifications and/or specialized training in dementia and/or memory support is a plus.
- Proven ability to lead employees in a positive, productive manner.
- Excellent interpersonal skills.
- Analytical & problem-solving skills.
- Strong ability to manage, multi-task and communicate effectively.
- Strong project management skills.
- Experience with monitoring and managing budgets.
- Proficiency in Microsoft Office software; EMR software (PointClickCare).
- Having previous experience in Skilled Nursing, acute hospital or similar healthcare settings is strongly preferred.
- Additionally, experience working in a Life Planned Community, continuing care retirement community, and/or Assisted Living facility is strongly preferred.
