We’re seeking a dedicated and experienced Home Care Coordinator to join our growing agency. This part-time, remote W-2 position supports both home care services and the launch of our Non-Emergency Medical Transportation (NEMT) program in Maryland.
You’ll play a key role in intake coordination, referral management, client scheduling, and community outreach. The right candidate will have a background in home care, strong communication skills, and a passion for helping others access quality care.
Key Responsibilities:
- Respond to referrals and coordinate intake for new clients
- Schedule clinical assessments with agency nurse
- Assign and manage caregiver schedules
- Support planning and development of NEMT program
- Build and maintain relationships with referral sources and healthcare partners
- Assist with contract management for both home care and transportation services
- Maintain accurate records and ensure compliance with Maryland state regulations
- Perform other duties as assigned
Requirements:
- Must reside in Maryland
- 2+ years’ experience in home care coordination, healthcare, or client services
- Familiar with Maryland Medicaid waivers, home care standards, and referral systems
- Strong communication, organizational, and follow-up skills
- Comfortable working independently in a remote setting
- Proficient with Microsoft Office, Google Workspace, and virtual meeting tools
Preferred:
- Experience supporting NEMT or transportation coordination
- Background in healthcare outreach or marketing
- Experience with EVV systems or caregiver scheduling platforms
- Bilingual is a plus
