What We’re Looking For
We are actively seeking a dedicated and experienced Clinical Supervisor for our Home Health Agency to support our Director of Clinical Services in overseeing clinical operations and compliance across our Skilled Home Health division.
This leadership role is ideal for a nurse (RN or LPN) with a strong home health background and experience as a Clinical Supervisor, Assistant Director of Nursing, Director of Nursing or Clinical Administrator in a Home Health Agency.
Key Responsibilities
- Provide day-to-day clinical oversight and leadership to the nursing team
- Support hiring, onboarding, evaluating, and mentoring clinical staff
- Ensure adherence to state, federal, and Medicare compliance regulations
- Collaborate on care planning and staff development
- Assist with and lead QAPI initiatives as Quality Improvement (QI) Coordinator
- Occasionally accompany or cover for field staff during patient visits (within Maricopa County)
- Work onsite in our Central Phoenix office with flexibility to travel locally as needed
Qualifications
- RN or LPN license (AZ or Compact) – Required
- Minimum 5 years of Home Health experience – Preferred
- OR 2 years as a Home Health Clinical Supervisor – Required
- Experience with AXXESS EMR – Preferred
- Valid AZ driver’s license, insurance, and reliable transportation – Required
- Must pass a background check
