The Urban Mission is looking for HRSN Navigator- come join our team as we continue to make a difference in our community! Looking for dedicated and professional Health Related Social Needs Navigators to join our team. In this role, and working through the Social Care Network, you will provide referrals to community members to improve social needs and promote better quality of life. You will play a crucial role in maintaining a welcoming environment while assisting the community. The ideal candidate will possess strong customer service skills and the ability to manage multiple customers in a nonjudgmental setting.
Responsibilities
- Once screened, assist and refer community members to appropriate social care needs, ensuring optimum, impartial professionalism
- Maintain accurate records and follow up as needed.
- Collaborate with team members to enhance and improve screening processes.
- Stay updated on relevant policies and procedures to ensure compliance with Social Network standards.
Qualifications
- Associate's Degree in Human Services or related field. Lived experience desirable.
- Previous experience in case management with the ability to provide excellent customer service is preferred, showcasing strong interpersonal skills.
- Ability to bill for Medicaid services
- Accurate note taking of customer screenings/files.
- Ability to work in a fast-paced work environment while maintaining attention to detail and accuracy.
- Valid Driver's license with ability to travel the Tri-County area
