Job Overview
We are seeking a dedicated and compassionate Patient Advocate to join our healthcare team. The ideal candidate will serve as a liaison between patients, families, and healthcare providers, ensuring that patients receive the highest quality of care. This role requires a strong understanding of medical terminology, and patient services to effectively navigate the complexities of healthcare systems.
Summary of Duties
Overall duties are to serve as a source of inspiration and motivation to HIV positive consumers enrolled in ASC’s Ryan White Part A Supportive Counseling Services program leading to the development of HIV self-management and self- advocacy skills.
The Patient Navigator is incorporated into the program to address program barriers, promote systems of care, and navigation and engage HIV positive individuals in care. The Patient Navigator is expected to provide specific services (below) that will enhance consumer support. The Patient Navigator will assist with case management and psychosocial services to engage consumers who are resistant to or sporadically engaged in their healthcare or treatment adherence and assist with support group services by sharing insight and personal experience.
Major Duties and Responsibilities
Facilitate program activities (accompaniment, client assistance, and re-engagement) that lead to enrollment and retention in the program.
Improve the coordination of care between subrecipient staff and other service providers.
Assist with referrals and linkages to care.
Assist Case Managers with interventions as needed.
Coach consumers in self-advocacy and self-management.
Maintain regular individual contacts with consumers, per program parameters. Support immigrant consumers in adjusting to the New York City healthcare system. Remind consumers of appointments.
Encourage consumers to participate in other life improvement activities such as nutrition, exercises, ESL classes, and support groups.
Participate in support groups sessions.
Fluency in on eof more major African language preferred.
Job Type: Full-time
Pay: $43,000.00 - $47,000.00 per year
