Position Title: Residential Liaison (Part-Time)
Salary Range: $30,000 – $35,000 annually (Salaried)
Reports To: Executive Management Team
Work Arrangement: 85% Remote | Field Visits Required
Position Summary:
The Residential Liaison serves as a key bridge between residential operations and administrative oversight at Heart Filled Homes. This dynamic, part-time role is ideal for a well-organized, proactive, and compassionate individual with a strong background in leadership and a passion for supporting individuals with unique abilities. The Liaison will oversee monthly monitoring of residential homes, support client transitions, collaborate with internal and external teams, and coordinate engaging events for our clients.
Key Responsibilities:
Residential Oversight & Coordination:
- Conduct monthly home monitoring visits across all Heart Filled Homes residential sites.
- Act as a point of contact for Residential Coordinators regarding concerns, needs, and operational support.
- Ensure logs and ledgers are up-to-date and printed monthly, including:
- Passdowns
- Homemaker Personal Care documentation & transportation logs
- Debit, EBT, and cash ledgers
- Behavior tracking logs (as needed)
- Staff medication control sign-off sheets (as needed)
Resident Lifecycle Management:
- Search the County website for potential new residents.
- Coordinate and conduct intake processes and move-ins for new clients.
- Organize and lead home tours for potential residents.
- Manage client acquisition and discharge processes.
- Maintain regular communication with Support Administrators and be available for ISP meetings.
- Lead move-out prevention efforts and monthly client satisfaction checks.
- Investigate motor vehicle accidents involving clients or staff.
Team Support & Supervision:
- Conduct 30-day and 90-day employee reviews with Residential Coordinators for new hires.
- Collaborate with management for continuous quality improvement.
- Attend one in-person management meeting monthly and a virtual meeting weekly (Mondays).
- Assist with ongoing training classes & new employee orientation.
Transportation Oversight (NMT):
- Fill in for Non-Medical Transportation (NMT) routes when regular drivers are off.
- Conduct physical ride-alongs every other month with NMT routes.
- Perform one incognito observation per NMT route monthly.
Community & Event Engagement:
- Plan and execute quarterly company outings for residents.
- Organize company-wide events including seasonal picnics and holiday parties.
Requirements:
- Preferred: Supervisory or management experience and prior experience managing transportation or NMT operations.
- Minimum 5 years of experience managing people OR a Bachelor’s degree in a related field.
- Strong organizational and leadership skills.
- Ability to work independently and multitask effectively.
- Passionate about serving individuals with developmental and intellectual disabilities.

- Completion of DODD Initial Training and ability to obtain a certificate.
- Must successfully complete and pass a background check.
- Must have a valid driver’s license with no more than two (2) active points.
- Flexibility to work some evenings/weekends if the task requires it.
