THE FORUM is recruiting for a hospitality focused C.N.A to join our team in the Memory Care Department.
Employment Type : Full Time
Shift Schedule: NOC Shift ( 11:00pm-7:00am)
Hourly Wage: $25.00- $31.73
Shift Differential: $3/hour
Sign on Bonus: $1500( see hiring manager for more details of the payout)

Under general supervision, provides personal care and supervision to residents in a manner conducive to their safety, comfort, security, and the greatest degree of independence possible. Provides direct resident care and assists in residents’ Activities of Daily Living (ADL) and scheduled Memory Care Program activities as a Universal Worker. Develops awareness of residents’ interests and needs and supports residents to thrive in their home and maximize their independence and participation. Actively incorporates the Memory Care Program, “Heartfelt Connections” model, Life Care Services’ (LCS) person -directed care philosophy and Dementia Care Standards into the residents’ daily rhythms of life. Duties and Responsibilities may be adjusted at the discretion of the Memory Care Manager.
Here are a few of the qualifications we need you to have:
- Valid C.N.A license in the state of CA.
- Ability to understand and follow instructions in English, communicates effectively, and performs simple arithmetic, and ability to document care given.
- High School Degree/GED required CPR/AED & First aid certification
