Overview
The Community Health Worker (CHW)/Social Worker (SW) plays a vital role in bridging the gap between healthcare providers and the community. This position involves engaging with individuals and families to promote health education, assist with navigating healthcare systems, and provide support for various health-related needs. The CHW will work closely with diverse populations to enhance access to healthcare services, improve health outcomes, and empower community members through education and resources.
Responsibilities
- Conduct outreach activities to engage community members and promote health programs.
- Educate individuals and families about available healthcare services, including Medicare options.
- Utilize motivational interviewing techniques to assess client needs and develop personalized action plans.
- Collect data on community health trends and individual client progress for reporting purposes.
- Provide information on nutrition, wellness, and disease prevention strategies.
- Assist clients in accessing resources for medical care, social services, and public health initiatives.
- Collaborate with healthcare providers, social workers, and other MIHP team members to coordinate care for clients.
- Maintain accurate records of client interactions and outcomes using medical terminology.
Requirements
- Previous experience in social work required, Michigan LBSW license.
- Familiarity with public health principles and practices is advantageous.
- Strong communication skills with an emphasis on public speaking and education.
- Experience in data collection methods related to health assessments or community surveys.
- Knowledge of motivational interviewing techniques to effectively engage clients in their health journey.
- A background in understanding aliment-related issues is beneficial for addressing client needs.

Pay: $22.00 - $25.00 per hour
