Position: Community Liaison/Healthcare Navigator
Community Liaison/Healthcare Navigator works closely with Veterans, their primary care providers, and members of the Veteran’s assigned interdisciplinary treatment team to ensure they have access to healthcare and benefits. The Navigator will also assist Veterans in navigating complex healthcare systems, including accompanying them to appointments and helping with benefit applications
Through the Supportive Services for Veteran Families (SSVF), a community-based outreach program, we identify low-income veteran households and connect them with a range of supportive services to promote housing stability and prevent homelessness. Location: Bronx, NY 10451
Minimum Qualifications:
- Associate degree, or 60 credits in Human Services or related field, or High School Diploma or GED with 1 year of experience in a healthcare or social services area of practice.
- Knowledge of the VA medical system preferred.
- Excellent communication and written and verbal communication skills, including basic computer skills.
1. Connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care.
2. Accompany Veterans to appointments at various institutions, including VA medical centers and other benefit institutions, to assist with accessing benefits and navigating healthcare systems.
3. SSVF Health Care Navigators provide case management and care coordination, health education, interdisciplinary collaboration, coordination and consultation, and administrative duties.
4. SSVF Healthcare Navigators work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team to develop relationships with community leaders, VA staff, and other referral networks.
5. The SSVF Health Care Navigator serves as a resource for education and support for Veterans and families and helps identify appropriate and credible resources and support tailored to the needs and desires of the Veteran, including regular review of care plan goals with the Veteran, conducting regular non-clinical barrier assessments, and providing resources and referrals needed to support adherence.
6. The Health Care Navigator will act as a liaison between the SSVF grantee and the VA or community medical clinic and work with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans.
7. Support outreach with screening and assessment efforts to identify veterans who may be eligible for SSVF services.
