Summary:
The RN Intake Coordinator serves as the initial point of contact for patients referred to healthcare programs or services. This role is responsible for evaluating referrals, performing detailed clinical assessments through medical chart reviews, and determining the appropriateness of patient admissions based on established criteria. The Intake Coordinator collaborates with internal teams, referral sources, and patients to ensure timely and accurate intake processes. The RN Intake Coordinator utilizes clinical judgment to identify patient needs, gaps in care, and potential barriers to treatment, providing essential information to care teams for safe and efficient transitions into appropriate programs. The RN Intake Coordinator plays a key role in facilitating access to care while maintaining compliance with organizational policies and regulatory standards.
Responsibilities:
- Serve as the initial point of contact for internal and external referrals, ensuring timely triage and coordination of program admissions.
- Collaborate with referring providers and partners to evaluate the clinical appropriateness of patients for targeted programs.
- Conduct thorough medical chart reviews to assess patients’ current and historical medical, psychological, and social backgrounds.
- Identify patient eligibility for specialized programs based on clinical criteria and documentation within the medical record.
- Recognize and document gaps in referral information or criteria that may affect program acceptance
- Extract and summarize key clinical data to support interdisciplinary treatment planning.
- Prepare and deliver concise, comprehensive patient summaries to the treatment team, including physicians, nurses, therapists, and ancillary staff.
- Act as a consistent liaison between referring sources and the treatment team to relay updates on patient status and progression.
- Maintain ongoing communication with patient families and multidisciplinary care teams to ensure clarity and alignment throughout the intake process.
- Educate patients and families on available care programs, services, and expectations to facilitate informed decision-making.
- Obtain prior authorization from insurance providers for planned admissions, ensuring compliance with payer requirements.
- Assess family readiness for treatment engagement through both in-person and virtual interactions.
- Coordinate with community-based organizations to enhance care continuity and provide feedback that supports resource integration.
- Lead daily bed control operations, managing patient flow and optimizing bed utilization to support clinical and operational goals.
- At least 3 years related experience Req
- Four-year Bachelor's degree or equivalent experience BSN Req
- Graduate or professional work or advanced degree; or equivalent experience Pref
- Current license to practice professional nursing in the State of Texas Upon Hire Req
- Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements if providing individual evaluations and/or group therapy 60 Days, Req
- Care coordination or insurance experience Pref
- Requires in-depth professional knowledge and practical/applied expertise in own discipline and basic knowledge of related disciplines within the broader professional field
