Summary:
The RN Intake Coordinator serves as the initial point of contact for patients referred to healthcare programs or services. This role is responsible for evaluating referrals, performing detailed clinical assessments through medical chart reviews, and determining the appropriateness of patient admissions based on established criteria. The Intake Coordinator collaborates with internal teams, referral sources, and patients to ensure timely and accurate intake processes. The RN Intake Coordinator utilizes clinical judgment to identify patient needs, gaps in care, and potential barriers to treatment, providing essential information to care teams for safe and efficient transitions into appropriate programs. The RN Intake Coordinator plays a key role in facilitating access to care while maintaining compliance with organizational policies and regulatory standards.
