Job Summary:
We are seeking compassionate, detail-oriented Housing Consultants to complete Housing Focused Person-Centered Plans (HFPCPs) as part of the HSS Housing Consultation service. In this role, you’ll work closely with clients who are navigating homelessness, unstable housing, or who live in settings that don’t meet their needs.
You will support individuals in identifying housing goals, navigating eligibility requirements, completing documentation required for HSS eligibility, and submitting these individuals for HSS Eligibility online. This is a hybrid position that requires in-person availability for the first two months, after which the schedule will shift to two in-office days (Monday/Wednesday) and three remote days per week.
Required Qualifications:
- 1–3 years of experience in human services, social services, or related fields (housing experience strongly preferred)
- At least 1 year of experience working directly with vulnerable and/or disabled adults
- Extreme attention to detail — this role requires consistent accuracy in documentation, paperwork, and form submission
- Highly proficient in professional communication via email, phone, and text
- Strong comfort level using documentation systems and e-signature software (e.g., DocuSign, Adobe Sign)
- Ability to manage a high volume of appointments, forms, and follow-ups in a fast-paced environment
- Self-starter who can work independently and efficiently while also collaborating with team members and supervisors
- Must pass a criminal background check
- Commitment to maintaining client confidentiality and following HIPAA and Minnesota Data Privacy regulations
Preferred Qualifications:
- Bachelor's Degree or equivalent experience
- Housing Stabilization Services experience (HIGHLY preferred)
- Strong organizational and time-management skills.
- Excellent communication and interpersonal abilities.
- Strong knowledge of housing resources and public benefits systems in the Twin Cities metro area
- Familiarity with Minnesota Healthcare programs is preferred.
Responsibilities:
- Complete upwards of 6-8 Housing Focused Person-Centered Plans (HFPCPs) daily with clients over the phone, virtually, or in person at our office
- Gather client data and supporting documentation needed for HSS eligibility, including Professional Statements of Need (PSNs) and MN–ITS verification for Medical Assistance coverage
- Document all client interactions and activities in our internal system in a timely and accurate manner
- Collaborate with referring providers, case managers, and social service agencies to ensure client success
- Educate clients on HSS program requirements, their rights, and the housing services available to them
- Maintain client confidentiality and follow HIPAA-compliant documentation standards
- Participate in regular supervision, team check-ins, and staff training
