GENERAL PURPOSE: The Intake & Stabilization Specialist is part of a multidisciplinary team that serves as the first point of contact for all incoming clients and clients’ family members. They assist with the client’s admission, record patient information, complete client assessments, as needed, and provide administrative support for other clinical and medical professionals. They support the client and their family during the stabilization and residential level of care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Greet clients upon arrival, facilitate a tour with the new client if requested, provide the client with information on the next steps during the admit process, and provide insight into the structure of daily programming.
- Greet families upon arrival, if new clients are transported via family members to the facility.
- Complete admission intake, this includes completion of clinical intake forms in KIPU.
- Coordinate property search with housing staff, according to the Search of Client Property SOP. Complete necessary documentation as needed.
- Check in with any detox or newly arriving residential clients twice per day, and more when needed, with appropriate documentation completed in KIPU.
- Address any needs for new clients by alerting the correct department of said client needs via chat, email, or direct one on one conversation with the appropriate staff member.
- Assist in relocation of patients from stabilization to RTC, including informing family of transition and transfer information to Primary Therapist.
- Supplying new clients with journals, writing instruments, and programming folders.
- Audit ROI’s for Intake, to ensure that client’s ROIs are complete and include a primary point of contact.
- Provide clients with an overview of PRC Programming as well as the community structure, expectations and requirements for utilizing services.
- Assist clients with reviewing and signing necessary consents and authorizations upon admission, ensuring all required documentation is completed accurately and in accordance with facility policies.
- Complete appropriate clinical assessments and documentation as needed, including the biopsychosocial assessment, client's initial treatment plan, and level of care determinations such as ASAM and/or LOCUS.
- Assign clients iPads and Fitbits during the admission process, ensuring proper setup of the client’s Peaks Portal to support engagement in programming and communication.
- Observe client behavior to identify change or indicators of special needs and share those observations with designated staff in order to carry out a necessary action plan.
- Attend daily rounds and provide updates to housing, medical, and clinical teams.
- Family Communication:
- Contact the family member or designated point of contact with a daily (or the time frame agreed upon by the family member/designated point of contact) with a progress update on the client.
- Send the introduction letter and invitation for family support groups.
- Provide support and guidance for family members who are struggling.
- Introduce family members, spouse, and/or partner to the concept of finding support for themselves.
- Collect necessary family information, such as diagnosis and trauma history, family relational issues and presenting concerns, and communicate with necessary team members.
- Document external contacts in KIPU daily.
- Monitor family phone for ongoing communication as needed for stabilization and residential clients
- Work with clients and their systems to identify family treatment goals and objectives.
- Maintain client charts/documentation according to the standards outlined in Clinical Records Management Policy & Procedure and the Assessment, Treatment Planning, Clinical Contact Notes Discharge and Aftercare Planning Policy & Procedure of PRC.
- Attend staff meetings, in-service meetings, peer reviews, supervision sessions, and other meetings as requested/required per week.
- Collaborate with the Medical Department, Nursing Department, and Case Management Department to coordinate client care plans as needed.

Education or Formal Training and Experience:
- Bachelor’s level degree in Social Work, Human Services, or Mental Health Counseling required.
- Certification Addiction Specialist licensure or working towards a higher licensure with the Colorado Department of Regulatory Agencies (DORA) is required.
- Current certificate for cardiopulmonary resuscitation (CPR)and first aid.
- Case management tracking and documentation experiences, preferred but not required.
Knowledge, Skill and Ability:
- Ability to learn is required.
- Interpersonal communication skills to develop and maintain effective working relationships with all internal and external customers by prioritizing customer service and customer satisfaction.
- Ability to organize and prioritize workloads in a sometimes hectic environment with frequent interruptions while remaining calm and tactful during stressful situations, emergencies and confrontations.
- Knowledge of community health and mental health resources and their eligibility criteria.
- Ability to work in a cooperative and collaborative manner with service agencies that are serving the client.
- Ability to follow and implement direction provided by the Program Director and/or Clinical Director, accepting supervision and feedback in a constructive manner.
- Working knowledge of HIPAA.
- Ability to use good judgment in order to maintain appropriate personal and professional boundaries with clients.
- Working knowledge of Google Suite.
- Ability to communicate efficiently and professionally, both verbally and in writing.
- Ability to produce written documentation in a manner readily understood by other service providers.
- Ability to relate in a supportive and non-judgmental manner to persons/clients in distress.
- Ability to make appropriate decisions in the absence of supervision.
- Basic keyboarding skills, including entering data in Kipu, PRC electronic health records.
- Ability to work independently and still work cooperatively/constructively as part of an interdisciplinary team.
- Ability to realize and activate potential in every interaction, every day.
- Ability to adapt to change in the workplace and to maintain a professional appearance appropriate for the position.
