Position Summary and Job Duties




Hours of work: Week 1- Sun, Tu-Fr and Week 2- M-TH, Sa 1pm-930pm.

Provides person-centered behavioral health assessment, care, resources and education to individuals who are experiencing or are at risk of experiencing a behavioral health crisis; 90% or greater time will be dedicated to working with the Beloit Police Department (BPD) as an integrated member of the CIT team; works to provide assessments, education, consultation and intervention to deescalate and/or resolve behavioral health crises in the community; stays connected with the crisis services unit through collaboration and is responsible for reviewing and documenting in Avatar; Collaborates with internal and external colleagues. Participates in clinical and administrative meetings. Participates in Police Briefings and CIT team meetings with the police department. Collaborates with internal and external colleagues. Participates in clinical and administrative meetings. Supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and “Commitment to My Co-Workers.”

80%- Provides person-centered behavioral health care services for clients experiencing a behavioral health crisis
1. Assess clients in crisis to determine least restrictive disposition that will assure client
safety
2. Assess for voluntary admission options for individuals without a funding source.
3. Authorize chapter 51.15 emergency detentions when appropriate.
4. Coordinate detox admissions under 51.45 with crisis services staff to an
appropriate receiving facility.
5. Coordinate admissions, when clinically indicated, to the appropriate receiving facility.
6. Coordinate medical clearance if required with police, referring facility, and hospital ER
staff.
7. Work with individuals and natural supports to create appropriate safety plans when
clinically indicated and in consultation with the crisis supervisor.
8. Create person-centered crisis response plans, crisis plans and updates
9. Coordinate client care and linkage BH services or resources as indicated
10. Provide outreach and prevention/early intervention to individuals who are high
utilizers of BPD services due to behavioral health concerns.
11. Provide clinical consultation to HSD and community partners as needed to support service coordination

10%- Ensure program and medical record compliance
1. Follow all departmental as well as divisional policies and procedures
2. Document client care in accordance with policies, procedures, and regulatory guidelines

5%- Participates in all clinical and administrative activities as required
1. Participate in case review meetings with HSD and BPD
2. Participates in crisis planning meetings
3. Participate in HSD and BPD staff meetings
4. Participate in meetings with supervisor as needed
5. Participates in Shift Report briefings with BPD

5%- Will perform other duties as assigned


Success Factors (KSA's)

Knowledge of the principles and practices of behavioral health care and its application, including person-centered care, trauma-informed care, co-occurring disorders, and stages of change.
Knowledge of laws, regulations, and practices pertaining to federal and state human service and behavioral health care programs
Knowledge of assessment and treatment of suicide, mental illness and substance use disorders
Knowledge of psychotropic medications, their main effects, side effects, and abuse potential.
Knowledge of psycho-social rehabilitation services
Ability to maintain accurate records and prepare clear and concise reports
Ability to establish and maintain effective working relationships within the agency and community.
Ability to understand and carry out verbal and written instructions
Ability to take initiative, work independently, and seek out supervision as needed
Ability to manage one's time and emotions in an environment of rapidly changing priorities
Excellent computer skills related to electronic record keeping, email, and resource identification.


Job Requirements, Education, Training & Experience

Masters degree in behavioral health field required;
LCSW, LMFT, or LPC preferred, or ability to obtain license within 3 years
Experience working with individuals with complex behavioral health needs;
Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto insurance pursuant to the Rock County Administrative Policy and Procedure 5.02 and 5.27.


Essential Job Functions (physical elements, equipment and working conditions)

Physical elements:
  • Ability to sit for long periods throughout the day
  • Some physical activity related to community based outreach duties

Equipment use:
  • Extensive computer including significant typing for documentation
  • Use of telephone, copier, and fax machine
  • Radio issued by BPD (training provided)

Working conditions:
  • Work is generally conducted in the community; BPD, emergency departments, on scene at a residence, or at
the crisis office
  • This position will require flexibility in scheduling.
