Summary
Advocate and coordinate Independent Living Services to individuals wishing to live independently within the community of their choice and empower those individuals to reach self-directed goals. Assist people in individual and group settings, both at the Center and in the individuals’ home and community in locating available resources. Flexible work hours and travel are necessary in performing job functions.

Essential Job Functions
- Provide the five core services: Individual and Systems Advocacy, Independent Living Skills Training, Peer Counseling, Information and Referral and Deinstitutionalization.
- Assist individuals with disability issues such as: Housing, Social Security, Medicaid, Transportation, the Americans with Disabilities Act, Deinstitutionalization and other disability related issues.
- Obtain and maintain Customer Service Records on all Customers served including Agency Customer packets and submit information to the Records Department.
- Develop and update Independent Living Plans and goals for each Customer choosing not to waive the Plan and maintain records to support activities and goals.
- Provide assistance in identifying and coordinating existing services, that will assist in achieving the IL goals established in the Independent Living Plan.
- Have a working knowledge of the American Disability Act (ADA) and be willing to train, counsel and advocate for ADA regulations.
- Identify barriers to integration within the community and actively pursue the removal of those barriers through the Agency’s ADA Advocacy Implementation Plan.
- Work with Vocational Rehabilitation Counselors as needed and assist in development of ILP, and IWRP and achieving goals identified.
- Assist agency in identifying unserved and underserved populations and providing outreach as identified appropriate to those populations.
- Participate in Agency projects such as the Food Bank and the Community Living Preservation Project (furniture and household items) to enhance services and provide avenues for Customers to become and remain independent.
Job Requirements
- At least 18 years of age
- Have a valid Driver's License
- Be familiar with Independent Living philosophy
- Clear background checks
