We are seeking an experienced Intake / Scheduler to join our home health team. The successful candidate will be responsible for creating and managing patient documentation and schedules, ensuring that all services are provided in a timely manner, and providing support to the care team. The ideal candidate will have prior home health experience, excellent organizational skills, the ability to multitask, and a commitment to providing excellent customer service.
Responsibilities:
- Monitor Fax via emails, coordinate home health services for patients.
- Maintain patient records and ensure accuracy of information.
- Communicate with healthcare providers, patients, and family members to ensure timely and appropriate services are provided.
- Ensure compliance with all applicable regulations and laws.
