Description:
Responsible for managing all aspects of the patient intake process including managing the members of the intake team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.
- Maintains confidentiality of patient information
- Directs all daily patient referral and intake operations including providing assistance in the establishment and implementation of intake policies.
- Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
- When referral is received, the Intake Coordinator will run Medicare (ie:insurance) through “my ability.”
- The Intake coordinator will be responsible to follow the intake and admission processes as confirmed through Operational Policy No. 2-004 and 2-005.
- The intake coordinator will screen patient and inform patient that a referral was received and give name of ordering physician or hospital.
- The intake coordinator will always introduce self and state where they are calling from and verify patient’s address.
- The intake coordinator will directly review written documentation of the patient’s demographics, Medical H & P, and SOC order (via email and/or hardcopy) with Uni Care Clinical Manager for admission appropriateness and including the need for additional Community-Based Palliative Care Services.
- Attend and participate morning team meetings
- Communicates all relevant information to the Community Outreach representative as well as appropriate operations personnel
- Works with operations staff to ensure appropriate level of care and service is provided to client.
- Establishes and maintains positive working relationships with current and potentialreferral sources
- Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes.
- Gathers, collates, and reports referral statistics including key customer referral trends.
- Maintains comprehensive working knowledge of Uni Care Home Health contractual relationships and ensures that patients are admitted according to contract provisions
- Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Uni Care Home Health
