Summary
The Special Needs Intake Specialist plays a key role in facilitating access to homecare services for pediatric and adult clients with developmental disabilities and complex medical needs. This position involves conducting detailed intake assessments, verifying eligibility, obtaining appropriate documentation, and coordinating closely with families, physicians, and interdisciplinary care teams. The specialist ensures a seamless onboarding experience that aligns with OPWDD, Medicaid, and other regulatory requirements.
Responsibilities
- Conduct comprehensive intake assessments to determine client eligibility for homecare services, including those under OPWDD.
- Collect, review, and ensure completeness of medical documentation such as Face-to-Face (F2F) notes, care plans, LCED/NOD forms, and supporting diagnoses.
- Interview clients and/or caregivers to gather information regarding care needs, services requested, and household dynamics.
- Coordinate with physicians, hospitals, service coordinators, and care managers to obtain necessary documentation and clarify care needs.
- Verify insurance eligibility, including Medicaid, Medicare, and commercial plans.
- Collaborate with internal departments and external partners to facilitate disenrollment from Managed Care plans when necessary.
- Check provider credentials in PECOS and OPRA systems.
- Maintain timely and professional communication with families, referral sources, and care team members throughout the intake process.
- Uphold compliance with all state, federal, and OPWDD regulations and company policies.
- Perform additional related duties as assigned by leadership.
Requirements
- Minimum of 1 year of experience in home health, social work, or developmental disability services preferred.
- Prior intake or care coordination experience, ideally within a CHHA or OPWDD setting.
- Working knowledge of Medicaid, Medicare, managed care, and waiver programs.
- Familiarity with systems such as HCHB, Waystar, EPACES, and Allscripts.
- Excellent interpersonal, communication, and customer service skills.
- Highly organized with the ability to manage multiple cases and deadlines.
- Proficient in Microsoft Office and electronic medical record systems.
- Bilingual (Spanish or Chinese preferred).
- Empathetic, patient, and able to work collaboratively with diverse populations and interdisciplinary teams.
Education
High School Diploma or GED required; Associate’s Degree or higher preferred.
