Position Summary
The LVN Wound Care QA Nurse is responsible for ensuring quality wound care documentation, adherence to clinical standards, and regulatory compliance for all wound care cases within the home health agency. This position works in an office setting, collaborating with field clinicians to review, educate, and improve wound care delivery and documentation.
Key Responsibilities
· • Review wound care documentation, including orders, assessments, and visit notes, for accuracy, completeness, and compliance.
· • Audit and monitor wound care charts to ensure alignment with physician orders and wound protocols.
· • Provide real-time feedback to field clinicians on wound care documentation, treatment plans, and best practices.
· • Verify that wound care orders are obtained, signed, and updated as necessary.
· • Monitor wound photographs and ensure they are appropriately labeled and uploaded per policy.
· • Assist in the development and maintenance of wound care pathways and protocols.
· • Track wound healing progression and collaborate with clinicians on care adjustments.
· • Educate clinicians on wound care best practices, documentation standards, and agency policies.
· • Collaborate with QA/PI team on wound-related performance improvement initiatives.
· • Maintain knowledge of CMS regulations, accrediting body standards, and clinical guidelines for home health wound care.
· • Participate in case conferences and interdisciplinary meetings involving wound care concerns.
· • Serve as a resource for clinicians regarding wound care products, treatment protocols, and documentation standards.
Qualifications
· • Current LVN license in good standing (State of practice).
· • Minimum of 2 years wound care experience; home health preferred.
· • Knowledge of CMS home health regulations and Conditions of Participation.
· • Strong documentation review and auditing skills.
· • Excellent communication and collaboration abilities.
· • Wound care certification preferred (e.g., WCC, LWCC) or willingness to obtain.

Work Environment
Office-based, Monday–Friday 8am-5pm. Occasional field visits or in-service education as required. Collaborates closely with clinical and quality assurance teams.
