Job Title: Medical Records Clerk
Job Description:
We are seeking a organized Medical Records Clerk to join our team. The ideal candidate will be responsible for ensuring that patient records are accurately separated, retrieved, organized, and distributed in accordance with provider requests and legal requirements. This position requires attention to detail, the ability to handle sensitive patient information, and the capacity to multitask efficiently.
Key Responsibilities:
- Separation and Organization of Records:
- Review incoming records from various providers (e.g., cardiology, PCP) and separate them according to specialty or type of document (e.g., Echo reports, imaging, lab results, etc.).
- Organize and classify documents such as imaging reports, test results, and medical histories as required by the provider or request.
- Retrieving and Requesting Medical Records:
- Retrieve hospital records from Dignity Health, Banner Health, and other healthcare systems.
- Request missing records or imaging reports from hospitals or other providers when necessary.
- Processing Specialist and Imaging Requests:
- Request and retrieve specialist records, imaging reports, and lab results as directed by physicians and other medical staff.
- Scanning and Document Management:
- Scan a variety of patient documents into the system, including but not limited to PHQ-9 forms, EKG results, biometric forms, FMLA documentation, signed record requests from patients, Mini-Cog assessments, medical histories, medication lists, and orders.
- Ensure all documents are properly labeled and indexed in the electronic medical records (EMR) system.
- Distributing Medical Records:
- Send medical records to specialists, lawyers, disability agencies, and other external parties as requested.
Confidentiality and Compliance:
- Maintain confidentiality of patient records and information in accordance with HIPAA and company policies.
- Ensure all records are maintained and processed according to legal and regulatory standards.
Requirements:
- High school diploma or equivalent.
- Experience in medical records management or a healthcare setting is preferred.
- Strong attention to detail and organizational skills.
- Ability to handle sensitive information with discretion.
- Excellent communication skills and the ability to work with healthcare providers and other staff members, external organizations, and patients.
- EMR knowledge, preferably knowledge of Eclinical (ECW)
- ANd other administrative duties if needed
