Overview
The Medical Social Worker at Serenity at Heart provides psychosocial assessments, resource coordination, and emotional support to home care and behavioral health clients and their families. This role ensures clients receive the social, emotional, and environmental support necessary to thrive safely at home, prevent unnecessary institutionalization, and navigate complex health systems with dignity. Collaborating with healthcare teams to ensure comprehensive care, particularly in settings such as hospitals, nursing homes, and outpatient facilities.
Duties
- Conduct thorough assessments of patients' social, emotional, and environmental needs.
- Develop and implement individualized care plans that address the unique challenges faced by patients.
- Provide case management services to coordinate care among various healthcare providers.
- Offer psychotherapy and counseling to patients dealing with chronic illness or significant life changes.
- Facilitate communication between patients, families, and healthcare professionals to ensure a holistic approach to care.
- Maintain accurate medical records and documentation of patient interactions and progress.
- Conduct diagnostic evaluations to identify patient needs and potential barriers to care.
- Engage in research activities to stay informed about best practices in social work within the medical field.
- Advocate for patients’ rights and access to resources, including special education services for children with disabilities.
Qualifications
- Master’s Degree in Social Work (MSW) required
- LCSW, LCSWA, or LMSW preferred (supervision available for associate-level candidates)
- Minimum of 1 year of experience in a healthcare, behavioral health, or home care setting
- Working knowledge of Medicaid waiver programs and community resources in NC
- Valid driver’s license and reliable transportation
- Strong interpersonal, organizational, and documentation skills
- Passion for serving seniors, medically fragile clients, and underserved populations
Work Conditions:

Pay: From $36.87 per hour
