Job Summary
The Care Coordinator plays a vital role in managing and overseeing the care of residents in assisted living, nursing homes, and other long-term care facilities. This position requires a compassionate individual with experience in senior care, including memory care and dementia care. The Care Coordinator will be responsible for developing and implementing individualized care plans, supervising staff, and ensuring the highest quality of care for residents. The ideal candidate will have strong leadership skills and a background in operations management within the healthcare sector.
Responsibilities
- Develop, implement, and monitor individualized care plans for residents, ensuring their needs are met effectively.
- Supervise and train staff members to ensure compliance with care standards and best practices.
- Coordinate with healthcare professionals to provide comprehensive support for residents with developmental disabilities, Alzheimer’s disease, or other cognitive impairments.
- Manage daily operations of the facility, including budgeting and resource allocation.
- Conduct regular assessments of resident health and wellbeing to adjust care plans as necessary.
- Facilitate communication between residents, families, and staff to foster a supportive environment.
- Oversee medication administration and ensure proper documentation is maintained.
- Promote a safe and engaging atmosphere for residents through activities that enhance their quality of life.
Requirements
- Proven experience in assisted living or nursing home settings; familiarity with long-term care practices is essential.
- Background working with individuals with developmental disabilities or experience in memory care preferred.
- Strong leadership abilities with previous supervisory experience in a healthcare environment.
- Knowledge of operations management principles within senior care facilities.
- Excellent communication skills to effectively interact with residents, families, and team members.
- Financial acumen for budgeting and resource management is desirable.
- Experience in social work or home & community care is a plus.

Pay: $29.00 - $37.50 per hour
