Serve geriatric patients in providing high-quality care in a clinic setting and home visits. Must be a bilingual either in Vietnamese or Cantonese or Mandarin.
Responsibilities:
- Deliver patient care in the Visalia area.
- Conduct annual health assessments in our new wellness clinic and some home visits.
- Travel between locations (costs covered).
Patient Load:
- 7-8 patients/day in clinic
- 3-5 patients/day on home visit days

Requirements:
- NP license for California.
- Experience with geriatric patients and EMR systems (EPIC preferred).
