Nurse Practitioner
Job Description
POSITION: The Nurse Practitioner (NP) independently operates as a healthcare provider, delivering comprehensive wound care and primary care services to patients across diverse settings. These settings include patients' homes, nursing homes, and wound care clinics. The NP is responsible for assessing, diagnosing, and managing acute and chronic conditions, with a particular focus on wound management and general health maintenance. This position reports to the CEO.

KEY RESPONSIBILITIES
- Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests.
- Develop and implement individualized care plans for wound care and primary care patients.
- Provide advanced wound care treatments, including debridement, dressing changes, and infection management.
- Monitor and evaluate patient progress, adjusting care plans as necessary.
- Educate patients and caregivers on wound care techniques, preventive measures, and overall health maintenance.
- Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure coordinated and comprehensive care.
- Maintain accurate and timely documentation of patient encounters and treatment plans.
- Participate in quality improvement initiatives and stay updated on best practices in wound care and primary care.
REQUIREMENTS:
- Graduate of an accredited NP program (MSN or DNP).
- Current RN license in Ohio and NP certification.
- National Certification (ANCC or AANP), active DEA license, CPR and BLS certification.
- Minimum 2-3 years' experience in an NP role, preferably in Adult, Family, Acute Care, or Geriatric specialties.

Pay: $118,123.00 - $127,970.00 per year
