About the Role
As a Qualified Developmental Disabilities Professional (QDDP), you will oversee the planning, coordination, and compliance-related supports for adults living in our licensed group homes. You will serve as the lead case manager for 25–30 individuals, partnering with families, CSBs, house managers, and clinical teams to ensure every person receives exceptional care aligned with their ISP.
Core Responsibilities
- Develop, update, and monitor Person-Centered Individual Support Plans (ISPs)
- Review and audit daily documentation, goal tracking, MARs, and behavior data
- Submit CHRIS incident reports on time and track follow-up activities
- Train and coach Direct Support Professionals (DSPs) and house managers
- Participate in licensing audit prep, documentation clean-up, and file compliance
- Conduct weekly site visits to monitor support quality and reinforce accountability
- Collaborate with support coordinators, nurses, behavior specialists, and families
- Respond to urgent behavioral or medical situations as part of an on-call rotation
Ideal Candidate Will Have:
- Bachelor’s or Master’s in Social Work, Psychology, Human Services, or related field (required)
- Minimum of 3 years of experience supporting individuals with ID/DD (required)
- Solid understanding of DBHDS licensing, CHRIS, person-centered planning, and Medicaid Waiver documentation
- Strong writing and auditing skills, with a keen eye for compliance
- Experience with electronic health systems (e.g., Therap, Foothold)
- Calm under pressure with excellent communication and training abilities
Compensation & Perks
- $65,000–$75,000/year, based on experience and caseload
- Candidates with 5+ years in Virginia often qualify for the top range
