Job Summary:
The Quality Improvement Associate works in collaboration with the Director and Team Lead of Quality Improvement to support opportunities for improved quality performance. Provides support to the physicians’ offices for patient outreach to schedule important visits to meet quality performance targets. Must possess the ability to work independently with strong organizational, communication, and interpersonal skills to support the management of multiple priorities and multiple locations, with significant attention to detail for completion of verbal and written communications.
Duties & Responsibilities:
Member Outreach and Scheduling Support
- Completes outbound calls and texts members for scheduling preventive visits and tests, aiding in care gap closure and quality performance improvement embedded at provider offices.
- Collaborates with provider offices or their designee as needed to coordinate member scheduling for member visits.
- Identifies and reports issues related to scheduling members or scheduling reports to the Quality Team Lead.
EMR Review and Data Management
- Utilize physician EMRs to verify completed visits and schedule for future visits.
- Maintains accurate scheduling and outreach data in working reports or data aggregator for outcome tracking and reporting.
- Ensures that all data and Protected Health Information (PHI) is maintained, transmitted, and stored in a confidential and secure manner applicable to organizational policies and procedures and according to state and federal regulations.
Quality Improvement and Collaboration
- Works collaboratively with physician-facing team to highlight relevant information that supports quality initiatives.
- Demonstrates ability to contribute to continuous quality improvement efforts in outreach and performance tracking.
- Understands clinical assessments, treatment protocol and core knowledge of quality improvement.
Communication and Reporting
- Provides regular verbal and written updates to Team Lead, Director, or other leadership on provider engagement and outreach efforts.
- Participate in work-related meetings outside of regular hours as needed.
- Performs other tasks or duties as assigned by management.
Educational / Experience Requirements:
- Strong Clinical background required.
- MA or LVN preferred.
- Bilingual- Speaks Spanish proficiently required
- Three to five years clinical or quality/performance improvement experience required
- · Understanding of clinical assessments and treatment protocols, knowledge of principles related to quality improvement.
- Requires knowledge of managed care contractual terms and operational issues.
- Demonstrated ability to manage continuous quality improvement initiatives through member outreach support.
- Strong verbal and written communication skills and problem-solving skills.
- Requires flexibility, to be able to switch priorities in positive and effective manner when directed by leadership.
- Strong working knowledge in Microsoft Office applications, Word, Excel, Outlook and PowerPoint; EMRs/EHRs.
- Must maintain valid Texas driver’s license and vehicle and automobile insurance coverage.

Physical Demands / Working Conditions:
- Travel to member offices is mandatory
- Ability to travel by car and transport required mobile equipment
- No supervisory responsibilities
