The RN Nurse Trainer is responsible for overseeing nursing services across HopeWay Community Services’ New Hampshire programs. This includes training employees and Home Care Providers to ensure compliance with state regulations and best practices. In collaboration with residential staff, the Nurse Trainer ensures safe medication administration, promotes professional development, upholds accountability, and supports high-quality care for adults with developmental disabilities and acquired brain disorders. Additionally, the Nurse Trainer provides ongoing oversight of client medical care and acts as a resource for medication education and guidance for staff, providers, and families.
Regulatory Compliance & Reviews
- Train and certify direct support staff in safe medication administration per He-M 1201
- Conduct quality assurance audits and support compliance with state regulations
- Support certification preparation and He-M 1201 reporting and follow up as needed
- Complete 5-day and 30-day nursing visits for individuals new to services
- Review and sign health histories within 30 days of admission and annually during Service Agreement reviews
- Ensure monthly HRST Tracker completion and maintain signed documentation annually
- Develop and implement individualized healthcare protocols under He-M 1001 and/or He-M 507
- Monitor progress toward annual health goals using the Annual Health Screening Recommendation form
- Participate in annual ISAs, quarterly team meetings, and monthly DDNNH nurse meetings
Clinical Expert & On-Call Triage Support:
- Provide triage support (phone/in-person) for staff addressing medical concerns and emergencies
- Collaborate with pharmacies and healthcare providers to ensure timely medication access
- Support diagnostic assessments and ongoing healthcare planning
- Coordinate with hospital staff during inpatient stays and contribute to discharge planning
- Deliver specialized training on high-risk conditions, including: Diabetes, Dysphagia & Aspiration, Seizures, G-Tube Care, Dementia, Coumadin, Clozaril & Lithium, Suprapubic Catheters, and Wound Care
Qualifications:
- Active and valid Registered Nurse (RN) license in the State of New Hampshire
- Minimum of two years of nursing experience in a human services, hospital, rehabilitation, or educational setting
- Valid NH driver's license and reliable transportation
- Active professional liability insurance
- Pass NH background and DMV checks as well as BEAS and TB testing
- Completion of all state and agency mandated training within 90 days of employment.
- Candidate must reside in the State of New Hampshire (Southern NH preferred)
Job Type:
- Full Time: $70,000 - $85,000 annually (based on experience)
- Part time: Up to $45 per hour (based on experience)
