Job Summary
The Social Services Resorce supports nursing home facilities throughout the Denver Metro and Northern Colorado regions to ensure compliance with state and federal regulations, including CMS and CDPHE standards. This role serves as a key resource for facility leadership and staff, offering guidance, training, and support to promote regulatory adherence and improve resident care outcomes. The ideal candidate will possess a strong background in social work, crisis management, and behavioral health, with a commitment to enhancing the quality of life for our clients.
Responsibilities
- Develop and implement social service programs that address SNF/LTC needs and enhance client well-being.
- Support facilities in assessments to identify client needs and develop individualized service plans.
- Assist facilities in preparing for annual surveys, audits, and inspections
- Provide technical assistance and interpretation of regulations related to resident care, documentation, staffing, infection control, and resident rights
- Develop corrective action plans in collaboration with facility leadership and monitor implementation progress
- Support facilities in improving quality metrics and survey readiness through training, coaching, and policy review
- Maintain current knowledge of applicable laws, rules, and industry best practices in long-term care
- Track and report on trends, deficiencies, and support needs across facilities to inform regional strategy
Skills
- Strong background in social work with experience health settings prefarably Skilled and LTC.
- Licensed Social Worker (LSW or LCSW) in the State of Colorado (Required)
- 3+ years of experience in long-term care settings, preferably in SNF/LTC
- Deep understanding of CMS regulations, CDPHE rules, and long-term care operations in Colorado
- Experience with survey processes, citations, corrective action plans, and quality improvement
- Strong interpersonal, analytical, and training skills
- Ability to travel regularly across the Denver Metro and Northern Colorado regions
- Valid Colorado driver’s license and reliable transportation required
- Proficient in Microsoft Office and electronic health record systems (e.g., PointClickCare)
- Strong organizational and documentation skills

Pay: $80,000.00 - $100,000.00 per year
