In this fast‑paced role, you’ll be serving as an advocate for patients and the broader community. You’ll facilitate timely referrals when care falls outside of Clinica’s scope—helping ensure underserved individuals are not left without access to essential services.

What You Get to Do:

  • Partner with providers and medical assistants to coordinate referrals that connect underserved patients to essential care such as physical therapy, vision services, and orthopedic specialists.
  • Carefully assess insurance coverage, patient location, and urgency to remove barriers and ensure timely access to care.
  • Send referrals securely and accurately, so outside providers have what they need to treat patients.
  • Support care teams by promptly prioritizing and responding to referral requests, both in person and through instant messaging.
  • Keep patient charts complete and up to date by scanning, documenting, and organizing intakes and referrals.

Compensation: Approximately $21.00 - $26.60 per hour. All individual pay rates are calculated based on the candidate's experience and internal equity.

  • Professional communication skills when interacting with patients, hospitals, and team members.
  • Ability to work collaboratively to support patients and achieve shared goals.

Compensation: $21.00 - $26.60/hour. The posted range represents the full salary range for this position. Starting pay is based on factors such as directly related experience, education, licensure and/or certification, and internal equity.
