Department:

Targeted Case Management (TCM)

Position Title:

Targeted Case Manager

Reports to:

· Director of Targeted Case Management

Supervises:

· NA

Qualifications

· Minimum of 21 years of age

License/

Certification:

· Maintain Abuse/Neglect/Exploitation, Ethics, Sexual Harassment, Emergency Preparedness, Vehicle Orientation, HIPPA, Bloodborne Pathogens and OSHA trainings and CPR, First Aid and Mandt certifications

Education:

· Minimum of High School Diploma/GED

· Prefer Bachelor degree in a related field with a minimum of 1 year related successful work experience

· Prefer Associate’s degree with related experience can be substituted if approved by management

Position

Summary

· Lead the day to day operation of Targeted Case Management Services

· Assume the responsibility of keeping a case load

· Stay current with legislative or regulation changes that affect TCM

· Lead the team toward achieving the BEST quality services possible

Experience:

· Minimum of 6 months in the Intellectual and Developmental Disabilities (IDD) field

Skills

· Excellent oral and written communication skills

· Excellent organizational skills

· Attention to detail to ensure task completion

· Possess good time management skills

· Ability to multi-task

Working

Conditions:

· 95% indoor

· 5% outdoor

· Duties are normally performed in a climate-controlled office

· May have high noise levels due to telephone calls, walk in clients, or other agency business

· Temperatures will reflect seasonal changes at outdoor sites and attempts to maintain comfortable temperatures will be made at indoor sites

· Noise levels vary by site but are usually moderate, although at times may be loud

· Environmental conditions vary by work site and employees are required to work at community locations

· Must follow assigned work schedules that typically reflect hours Monday-Friday. Schedules and hours vary based on need

· Employees are expected to remain flexible and cooperative.

Requirements

· Regularly required to stand, kneel, stoop, squat, talk, walk, and hear

· Occasionally required to reach with hands and arms

· Frequently required to sit

· Frequently required to use hands to finger, handle, feel objects, manipulate tools, or controls and grasp objects

· Ability to listen and remember information

· Ability to speak clearly

· Regularly lift and/or move up to 15 pounds
