Are you an experienced Home Health or Hospice RN leader ready for an exciting opportunity to make a difference?
Always Best Care Senior Services is seeking a knowledgeable, motivated Director of Patient Care Services (DPCS) to lead our Medicare-certified, accredited Home Health division serving Solano and Napa Counties.
We're looking for a confident clinical leader who understands regulatory compliance, accreditation, and survey readiness—and enjoys building strong teams and efficient clinical operations.
What You'll Do
- Lead clinical operations and supervise our home health care team.
- Ensure compliance with Medicare Conditions of Participation, California Title 22, and accreditation standards.
- Oversee survey readiness, QAPI, clinical audits, and Plans of Correction.
- Monitor OASIS, clinical documentation, care plans, and quality of patient care.
- Collaborate with the Administrator to strengthen clinical operations and support agency growth.
What We're Looking For
Required:
- Current, unrestricted California RN license.
- Must meet California Title 22, Section 74703 DPCS
qualifications
Bachelor's degree or higher in nursing or a health-related field: 3 years of qualifying experience within the past 5 years, including 1 year in supervision or administration; OR
- RN without a qualifying bachelor's degree: 4 years of qualifying experience within the past 5 years, including 1 year in supervision or administration.
- Qualifying experience must be in a home health agency, primary care clinic, or health facility.
Home health or hospice clinical leadership experience.
Experience with accreditation or regulatory surveys and clinical compliance.
Strong organizational, communication, and leadership skills.
Preferred:
- Previous DPCS, DON, or Clinical Director experience.
- Experience with ACHC, CHAP, The Joint Commission, or CDPH surveys.
- Medicare home health knowledge, including OASIS and QAPI.
- WellSky Home Health experience.
