Promptcare is seeking a compassionate and driven Community Resource Coordinator (CRC) to join our team, supporting individuals living with bleeding disorders. Job Type: Full-time, 40hrs per week, Monday-Friday, hybrid.
Location: You must reside in the Central or Eastern time zone.
Requirements
- High School Diploma or equivalent required
- 2–5 years of experience in a healthcare setting preferred
- Familiarity with national and local bleeding disorder chapters and available assistance programs is a plus
- Strong event planning skills, including the ability to manage logistics, budgets, and timelines
- Experience coordinating with clients, vendors, and cross-functional teams
- Critical thinking skills and a solid understanding of social services, as well as public and commercial health insurance
- Demonstrated commitment to providing excellent customer service
- Ability to manage multiple priorities in a fast-paced, dynamic environment
- Exceptional communication and interpersonal skills
- Valid driver’s license and auto insurance required
- Willingness to travel up to 20% of the time
Job Responsibilities, included but not limited to:
- Build and maintain relationships with bleeding disorder patients to support therapy adherence, retention, and overall satisfaction
- Conduct regular outreach and in-person visits to assess needs, provide education, and coordinate resources
- Serve as a liaison between patients, nonprofit partners, and internal teams to ensure comprehensive support
- Participate in community outreach and patient engagement events
- Collaborate with multidisciplinary teams to address social service and insurance-related needs
- Research and maintain knowledge of community resources and bleeding disorder support programs
- Assist with medication and supply coordination, including occasional deliveries
- Plan and execute patient-focused events and represent the company at local and national bleeding disorder chapter events
- Coordinate event logistics, budgets, and vendor relationships
- Stay current on bleeding disorder therapies and community developments
- Perform additional duties as assigned
