Job Title: Bilingual Case Manager
Fulltime/40hour week/with Benefit
Reports To: Case Management Supervisor / Program Director
Basic Function: Assist clients with counselling, entitlement services, following up with medical and other appointments, and housing referral. Help individuals with their un-met needs and make the necessary referrals when warranted. Approach and counsel residents holistically for them to maintain their housing and adhere to the Program’s House Rules and housing agreement.
Responsibilities:
- Responsible for carrying a caseload of about 20. Meet with residents on a weekly basis or more depending on resident’s needs.
- Give on-site counseling to residents and provide crisis intervention counseling.
- Advice and counsel residents regarding unmet needs.
- Develop individual care plans, make referrals, and obtain entitlements for clients.
- Conduct surveys and prepare reports.
- Document all work, including progress notes.
- Attend staff and other agency meetings. Participate with Program OMH audits.
- Abet agency with rent collection and follow- ups.
- Prepare paperwork for applicants admitted into the program.
- Engage residents in recreational activities.
- Aid office personnel as needed.
- Complete assigned tasks as delegated by the supervisor, Director, and CEO
Qualifications:
BA/BSW Degree in social Work or related field. A Minimum of 2 years’ experience working with the mentally ill/ homeless / Substance Abuse Population in a residential setting is required. Good interpersonal and written communication skills, Computer Literate.

Pay: $45,000.00 - $50,000.00 per year
