Job Summary
The Family Coordinator is responsible for serving as the primary liaison between the organization, adults with disabilities, and their families or support systems. This role ensures that families are informed, engaged, and supported in navigating available resources, services, and programs. The Family Coordinator works collaboratively with staff, community partners, and external agencies to enhance the well-being and independence of individuals with disabilities.
Tactical goals of this position include:
· Start and maintain daily a closed FB page for families or other social media as appropriate
· Contribute to the Lochland Facebook page and other social media on a weekly basis
· Monthly digital Family newsletter
· Monthly online sessions for families
· 4 large family/ Lochland community events during the year
· Support the residents in smaller groups to attend local events including arts and culture with families
· Provide written updates to each family on a schedule determined by each family
· Attend care coordination meetings
· Provide enhanced support for families of new residents transitioning into Lochland
· Act as a point of contact for Lochland families

Duties & Responsibilities
· Act as a point of contact for families and support system, providing guidance, resources, and advocacy.
· Develop and maintain relationships with families to ensure they are engaged and informed about services.
· Coordinate and facilitate family support groups, workshops, and informational sessions.
· Collaborate with service providers and other professionals to support individualized care plans.
· Maintain accurate and confidential records of family interactions, concerns, and support provided.
· Advocate for the needs and rights of adults with disabilities and their families.
· Develop and distribute newsletters, emails, and other communications to keep families informed.
· Organize and participate in community outreach events to raise awareness and support for the organization’s mission.
· Assist in program evaluation and development based on feedback from families and individuals served.
- Assist in the houses as necessary to support daily operations and ensure smooth functioning.
- Perform other duties as assigned.

Required Skills & Abilities
· Strong interpersonal and communication skills, with the ability to build trust with families and individuals.
· Knowledge of disability services, family support strategies, and community resources.
· Ability to advocate effectively for individuals with disabilities and their families.
· Excellent organizational and time management skills.
· Proficiency in Microsoft Office and database management.
· Ability to work independently and collaboratively as part of a team.
· Sensitivity and cultural competence in working with diverse populations.
· Ability to handle confidential information with professionalism and discretion.
· Passion for the mission of supporting adults with disabilities.

· Capability in social media and graphic design and/ or willingness to learn.
· Capability in developing content and layout for newsletters and social media.
· Willingness to pick up additional shifts to support residential programs.
Education & Experience
· Bachelor’s degree in social work, psychology, human services, or a related field preferred.
· Minimum of 2 years of experience in disability services, family support, case management, or a related role.
· Experience working with individuals with disabilities and their families is highly desirable.
· Preferred prior experience and knowledge of OPWDD.
· Experience in social media and online forums.
· Ability to complete SCIP, first aid, CPR and all other required trainings.
Physical Requirements
· Ability to work in an office setting as well as travel for community events and home visits.
· Ability to sit, stand, and move intermittently throughout the workday.
· Ability to lift up to 20 pounds occasionally.
· Must be able to navigate various departments of the organization’s physical premises.
Job Type: Full-time
Pay: $18.00 - $24.00 per hour
Expected hours: No less than 40 per week
