Western Tidewater Community Services Board (WTCSB) is seeking a motivated and compassionate Case Manager to support our School Counseling Intervention Program in Southampton County, which provides prevention, early intervention, and counseling supports to students experiencing academic, behavioral, or emotional challenges. The Case Manager will work collaboratively with school staff, families, and community partners to ensure students have access to the resources, skills, and supports necessary to succeed in both their educational and personal development.
Key Responsibilities:
- Provide case management services to students referred for counseling and intervention support.
- Conduct intake assessments, develop service plans, and track progress toward individualized goals.
- Collaborate with school counselors, teachers, administrators, and families to address barriers to learning and well-being.
- Facilitate linkage to community resources, including mental health, social services, and academic supports.
- Support program counselors by coordinating services, scheduling sessions, and monitoring student engagement.
- Maintain accurate documentation, case notes, and data in accordance with agency and school system requirements.
- Participate in school-based meetings, multidisciplinary teams, and case conferences as needed.
- Provide crisis intervention and de-escalation support when appropriate.
- Assist in delivery of psychoeducational groups, skill-building activities, and workshops.
- Uphold confidentiality and adhere to ethical and professional standards.
Documentation and Reporting
- Maintain accurate and up-to-date case files, including assessments, service plans, progress notes, and discharge summaries, in accordance with agency and HIPAA standards.
- Collect and analyze data to measure program effectiveness and report outcomes to stakeholders and funders.
- Ensure timely completion of required documentation and compliance with program standards
Qualifications:
- Bachelor’s degree in Social Work, Human Services, Psychology, Education, or related field (Master’s preferred).
- Minimum of 1–2 years’ experience providing case management or direct services to youth and families, preferably in a school or community setting.
- Knowledge of child and adolescent development, behavioral health, and family systems.
- Strong communication, organizational, and problem-solving skills.
- Ability to engage students and families from diverse cultural and socioeconomic backgrounds.
- Familiarity with local community resources and service systems preferred.
- Must pass background check and meet all school clearance requirements.
- Valid driver’s license and reliable transportation required.
