Centurion Residential is seeking a compassionate and organized Service Coordinator to support our residents’ independence, well-being, and quality of life. This role is essential to our commitment to aging-in-place and enriching the lives of older adults through connection to services, wellness programming, and advocacy.
Key Responsibilities:
- Assist residents with identifying and accessing public benefits, healthcare, transportation, and supportive services
- Coordinate wellness checks, health screenings, and educational workshops
- Develop and implement monthly calendars of programs, events, and group activities
- Establish relationships with local service providers and community-based organizations
- Monitor and maintain confidential case management notes in accordance with HUD standards
- Collaborate with property management, maintenance, and concierge staff to address resident needs and concerns
- Respond to emergencies involving residents and follow up as appropriate
Minimum Qualifications:
- Bachelor’s degree in Social Work, Human Services, Gerontology, or related field
- At least 2 years of experience working with older adults or low-income populations
- Excellent communication, organizational, and problem-solving skills
- Knowledge of Medicare, Medicaid, SNAP, transportation assistance, and aging services
- Ability to maintain professional boundaries and uphold resident confidentiality
Preferred Qualifications:
- Experience working in HUD-assisted or affordable housing settings
- Case management or care coordination background
- Familiarity with Yardi, Excel, and digital record-keeping systems
