Assist the Community Resource Director in overseeing the resource operations for Partners for Kids and Families (PFKF)
Help in the research and development of resources
Assist with obtaining new and prospective providers
Conduct provider visits
Collaborate with the Community Resource Director to maintain Memorandum of Understanding (MOU) with providers
Update SharePoint as it relates to resources. Organize and maintain SharePoint site for saving relevant documentation from community providers including but not limited to staff lists, liability insurance, and professional licenses.
Manage and maintain digital content displayed on office monitors, including uploading announcements, slides, documents, and videos.? Ensure content is accurate, up-to-date, and visually appealing
Collect, analyze, and generate data reports on the use and effectiveness of resource services.
Create and maintain tracking forms through Smartsheets to support the needs of the Bronze Team
Create and develop resource fairs and resource meetings
Provide consultation to families, CMO staff, and systems partners regarding access to available resources
Develop a quarterly resource newsletter
Establish and strengthen relationships with our system partners and inform them about Care Management Organizations and the Children's System of Care
Establish a positive presence in the community by participating in community based committees and meetings
Coordinate events, holiday donation drives, and programs for the agency and community
Assist with the modification and improvement of the Burlington Resource Net
Collaborate with Care Managers to reduce gaps in services
Help with creating resources for youth with intellectual and developmental disabilities
Aid in the process of assessing the need for and creating the use of community resource dollars
Work with the Community Resource Director to provide more social media exposure for PFKF
All other duties as assigned
The position starts at $55,000 or your current salary plus $4,000, whichever is higher. For those currently at a Care Manager Lead role, this would be considered a lateral move. They would stay at their current salary.
