Base Pay: $21.00 - $31.50 / hour
JOB SUMMARY
The Housing Navigator is responsible for working with unsheltered individuals living primarily in outdoor locations or other nontraditional living environments to reduce their barriers to housing and support their access to permanent housing opportunities that meet their needs. This position will work directly with the HOPE and Enhanced Patient Services Departments. Strong patient engagement skills are needed to engage with a complex and dynamic patient population. This position requires effective communication and the ability to work successfully as a team with internal and community partners. Strong Excel skills are helpful to manage the outreach activities driven by electronic data reports and client referrals. Other duties include helping patients with patient assistance programs; providing patient information about SCHC, other agency and community resources; and attending appointments with patients to support their engagement in housing.
JOB DUTIES AND RESPONSIBILITIES
- Outreach to patients identified by the HOPE Case Manager and Street Outreach Case Manager in need of housing.
- Develop and maintain a strong working relationship with identified patients.
- Coordinate with other professionals who help patients, such as social workers, SCHC Clinicians, and other local agencies.
- Connect patients to community resources as it relates to housing and barriers to housing.
- Complete with patients: Barriers to Housing assessments, HUD applications, and housing applications.
- Conduct data entry and ensure consistent and accurate data is captured in patient charts.
- As part of the HOPE and Enhanced Patient Services team conduct Outreach activities in various community locations.
- Promote SCHC programs and services to the community as needs arise.
- Enhance professional growth and development through participation in education programs, workshops, and in-service meetings.
- Self-directed and able to organize and manage multiple tasks.
- Perform other duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
- Knowledge of client engagement techniques such as Motivational Interviewing, Harm Reduction, Housing First Principles.
- Knowledge of Shasta County Housing resources.
- Ability to work in a variety of work settings.
- Strong communication skills.
- Moderate skills in a Microsoft Windows environment, including use of Microsoft Office suite (Outlook, Word, Excel, PowerPoint).
- Proficiency in verbal and written communication in English.
- Ability to read, write, understand and spell English and medical terminology correctly.
- Demonstrated ability to work effectively in a team environment.
- Moderate to advanced problem-solving skills.
- Sound judgment to support making appropriate decisions to benefit patients and meet SCHC objectives.
- Ability to prioritize tasks and complete on a timely basis with minimal supervision.
- Ability to deal with change and seek out opportunities to effect change to promote patient care.
- Strong customer service approach to problem solving situations.
EDUCATION & EXPERIENCE
- High school diploma or equivalent.
- Type 45 WPM or greater.
- Excellent people skills and customer service orientation.
PREFERRED QUALIFICATIONS
- 2 years of college education and experience in case management practices.
- Experience working with individuals who have a history of homelessness, substance use and mental health.
- Experience with electronic health records.
- Experience working with patients to connect to housing and community resources.
