Job Summary
We are seeking a dedicated and skilled Wound Care Nurse Practitioner to join our healthcare team as a traveling provider. The ideal candidate will have a strong background in wound care management and will be responsible for providing high-quality patient care in various settings. This role requires excellent clinical judgment, the ability to work collaboratively with other healthcare professionals, and a commitment to improving patient outcomes.
Responsibilities
- Must be willing to travel. Coverage is Uvalde, Del Rio, Eagle Pass, Kingsville and Corpus Christi.
- Conduct comprehensive assessments of patients with acute and chronic wounds.
- Develop and implement individualized care plans based on patient needs and evidence-based practices.
- Perform wound debridement, dressing changes, bio-engineered skin graft application and other wound care procedures.
- Monitor vital signs and assess patients for potential complications.
- Provide education to patients and families regarding wound care management and prevention strategies.
- Document all patient interactions accurately using appropriate coding systems, including ICD-10.
- Collaborate with interdisciplinary teams to ensure coordinated care for patients.
- Stay updated on best practices in wound care and participate in ongoing professional development.
Experience
Candidates should possess the following qualifications:
- Previous experience in wound care is preferred
- Proficiency in wound assessment is necessary
- Familiarity with coding practices related to wound care management.
- A strong background in adult practice, working Home Health, LTC settings and with dementia patients is advantageous.
